Community Health Assessment

The purpose of this community health assessment (CHA) report is to summarize important health issues that emerged from analyzing various data sources, including the voice of Douglas County residents, related to the following questions:
  • What are the health problems in a community?
  • Why do health issues exist in a community?
  • What factors create or determine health problems?
  • What resources are available to address the health problems?
  • What are the health needs of the community from a population-based perspective?

History

Virtual Public Health Improvement Plan (PHIP) Community Meetings

Community meetings on prioritized health topics were held in October and December 2021.  Data from the online survey and related meetings is available below:

Community Health Survey

Community Health Survey
The first step toward creating a CHA was a Community Health Survey, which gathered input from community members to help identify and understand opinions about health issues and the quality of life in Douglas County. The survey closed on Sept. 7, 2021. View a summary of findings in the Community Health Assessment Update.

Community Health Assessment Process

The information collected during phase one – the Community Health Survey – was compiled by HMA, who then conducted meetings across the County virtually and in-person. HMA shared the results from the survey and gathered additional input about county residents’ health priorities. The meetings were held in December 2021.

HMA used information learned from the survey and the community meetings to create the CHA, which they presented to the County. The priorities identified will be used to develop the Douglas County Public Health Improvement Plan (PHIP). The health improvement plan will aid the county in program planning and budgeting resources over the next few years.

Public Health Improvement Plan (PHIP)

Douglas County contracted with Health Management Associates, Inc. (HMA), a public health and health care research and consulting company to create a Public Health Improvement Plan (PHIP) specific to Douglas County and the public health needs identified in the CHA.
