Help identify Douglas County’s most pressing health issues

Douglas County residents are invited to participate in a Community Health Survey, available online through Sept. 7 at 9 p.m.

Your input is needed to help the County understand the public health services needs of its residents and business community.

Douglas County residents are invited to participate in an online Community Health Survey, administered by Health Management Associates (HMA).

Please complete the survey online in English or Spanish by 9 p.m. on Tuesday, Sept. 7, 2021.

Your answers will be used to help identify pressing health problems and issues and are vital to understanding our community’s opinions about the quality of life in Douglas County.

This Community Health Survey is the first phase of a process to create a Community Health Assessment (CHA)

Information collected through the survey will be compiled by HMA who will conduct meetings across the County, virtually and in-person, to share the results and gather additional community input. Your first opportunity to attend a meeting will be by phone or online during a Live Town Hall on Tuesday, Sept. 28 at 6:30 p.m.

The priorities identified by the survey and the community meetings will be compiled into the CHA, presented to the County and used to create a Douglas County Public Health Improvement Plan (PHIP) which will aid the County in program planning and budgeting resources in the future.

Don’t miss your opportunity to shape the future of Public Health in Douglas County – the survey will not collect any personally identifiable information and will take less time than drinking your morning beverage.

The survey is available in English and Spanish until Sept. 7 at 9 p.m. and can be completed on a computer, tablet or mobile device – visit douglas.co.us and search for “Community Health Assessment.”
